Position OverviewThe Health Record Technician I (HRT I) works in the Health Information Management Department (HIMD) under the supervision of the Health Record Technician II Supervisor and general supervision of Health Record Technician III Supervisor and HIMD Director.
Responsibilities- Abstracts and codes health records using ICD-10, DSM-5, HCPCS and CPT coding systems for fee-for-service and ancillary forms.
- Performs health record maintenance including filing, thinning, splitting, and destruction of records.
- Enters discharge, court-visit, and acute-leave patient data into Excel spreadsheets from the daily hospital census.
- Audits, logs, and tracks court-visit and discharge summaries for compliance with timeliness requirements.
- Maintains confidentiality, privacy, and security of PHI and PII in accordance with IPA, HIPAA, WIC, Title 22, and DSH policies.
- Demonstrates professionalism, teamwork, initiative, judgment, and responsibility, and complies with hospital policies.
- Reports to work on time and maintains a professional appearance.
- Completion of the Health Record Technician I Exam Bulletin and passing the required exam.
- Minimum experience and education as set out in the Class Specification.
- Background investigation clearance and medical clearance prior to hiring.
- Strong knowledge of medical terminology and health record abstracting and coding principles.
- Proficiency in Microsoft Word, Excel, PowerPoint, Outlook, and the ADT system.
- Effective communication skills and ability to interact tactfully with the public.
- Keyboard skills and familiarity with health record maintenance tasks.
