RN Care Manager: Complex Care & Coordination

LaSalle Medical Associates IPA is seeking a qualified RN Care Manager to deliver essential care management services to members with complex medical and behavioral health needs. This role involves outreach, member engagement, clinical assessments, and the coordination of care across various providers to ensure timely and patient-centered solutions.

The ideal candidate will have at least 3 years of care management experience, an ASN or BSN, and must possess an active California RN license. Understanding chronic disease management principles is vital, as well as strong interpersonal and organizational skills.
