Pay range (depending on experience): $55.00 - $63.00 hourly

• Initiate the plan of care and make necessary revisions as patient status and needs change.
• Use health assessment data to determine nursing diagnosis.
• Develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions. Include the patient and the family in the planning process.
• Provide health care instructions to the patient as appropriate per assessment and plan.
• Assist the patient with daily living activities and facilitate the patient’s efforts toward self‑sufficiency and optional comfort care.
• Act as Case Manager when assigned by Clinical Manager and coordinate patient care for assigned caseload.
• Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders as required.
• Teach the patient and family/caregiver self‑care techniques as appropriate. Provide medication, diet, and other instructions as ordered by the physician and recognize opportunities for health counseling with patients and families/caregivers. Work in concert with the interdisciplinary group.

• Graduate from an accredited school of nursing. Bachelor’s degree preferred.
• Current state license as a Registered Nurse.
• Minimum of 1 year of recent acute care experience in an institutional setting.
• Minimum of 1 year of recent experience in home care, preferably in hospice.
• Valid driver’s license and reliable, insured transportation.
