Registered Nurse

We are seeking a skilled and experienced Registered Nurse (R.N.) to join Alpha Care Home Health. The ideal candidate will demonstrate expertise in utilizing OASIS to admit, transfer, re-certify, and discharge patients according to physician orders. This role involves overseeing and coordinating the care provided by LPNs and HHAs, ensuring all patient care is delivered in compliance with agency policies and procedures.

The R.N. will also ensure adherence to Medicare and Medicaid regulations, while maintaining a high standard of care for all home health patients.

• Must be a licensed Registered Nurse (R.N.).
• Minimum of 1 year of nursing experience as an R.N. required.
• Ability to work collaboratively and positively with patients, families, and staff.

• Conduct initial home care visits and regularly reassess patient needs and progress.
• Develop and implement care plans based on physician orders.
• Complete OASIS assessments for admissions, transfers, re-certifications, resumptions of care, and discharges.
• Observe, assess, and document patient symptoms and progress.
• Monitor patient reactions and outcomes.
• Educate patients and caregivers on disease management, medications, care plans, and treatment.
• Instruct patients and caregivers in home healthcare techniques.
• Coordinate patient services and schedules.
• Supervise LVNs and HHAs in the delivery of patient care.
• Communicate changes in patient condition to physicians and appropriate clinical staff (Clinical Manager, Therapist, Case Manager).
• Execute skills as outlined in the agency's policy and procedure manual.
• Discharge patients from skilled nursing care when discharge criteria are met.
• Facilitate case conferences to ensure coordinated care.
• Maintain accurate and up-to-date clinical records in accordance with policies and procedures.
• Participate in continuing education, in-service training, and self-study programs to maintain skills and knowledge.
• Provide on-site supervision of LPNs and HHAs.
• Comply with all Medicare and Medicaid regulations.
• Perform other related duties as assigned.

Hourly Range: $55.00 - $75.00
