We're looking for a Care Coordinator to join our San Diego team. This is a hands‑on role at the center of our operations — you'll be the bridge between our clients, our caregivers, and our leadership team. From managing referrals and staffing HCAs to leading orientations and ensuring compliance, you'll play a critical part in keeping our agency running smoothly and our clients cared for.

Client Management & Staffing- Source and match Home Care Aides to clients based on skill level, certifications, personality, and availability
- Maintain accurate, up-to-date client schedules in our management system (Well Sky)
- Support new client onboarding including phone and in‑person referrals, virtual and in‑person consultations, and quality assurance visits
- Develop a strong understanding of common client comorbidities to ensure the right HCA is matched to the right care needs
- Serve as the primary liaison for our HCAs — build rapport, provide support, and advocate for their well‑being
- Conduct monthly quality assurance calls with HCAs on assignment
- Monitor safety concerns, complete required documentation, and collaborate with supervisors to resolve issues
- Communicate with clients and families regarding HCA performance, scheduling, and staffing changes
- Support the Talent Support Specialist with recruitment efforts — posting job ads, reviewing applications, verifying references, and conducting interviews
- Lead and support in‑person orientations for new hires, ensuring all required forms, background checks, and skills verifications are completed
- Create and maintain up‑to‑date personnel records including certifications, credentials, and performance reviews
- Ensure all HCAs meet state and agency certification requirements and that background checks are completed before placement
- Enforce adherence to agency policies and procedures, implementing corrective actions when necessary
- Document staffing events in Well Sky and ensure timely resolution of issues
Assist with day‑to‑day office operations — answering calls, managing supplies, greeting visitors, and supporting the team however needed - Work closely with the Executive Director and Administrator to keep operations running smoothly
- Cross‑train in other business areas to provide support as needed
- 1 year of office experience
- 1 year of home care or healthcare industry experience
- Proficiency in computer systems, data entry, and word processing
- Excellent customer service and interpersonal skills
- Ability to manage multiple priorities in a fast‑paced environment
- Live Scan Fingerprint Clearance (LIC 9163)
- Statement Acknowledging Requirement to Report Suspected Abuse (SOC 341A)
- I‑9 Employment Eligibility Verification
- Criminal background check
- Current TB clearance
- Experience with Well Sky, Salesforce, or similar care management platforms
- Familiarity with HIPAA regulations
- Prior experience staffing or scheduling in a home care setting
$28.00 – $30.00/hr based on experience
