The mission is to enhance and sustain the health and well‑being of lesbian, gay, bisexual, queer, transgender, nonbinary, immigrant, and HIV communities for the betterment of our entire San Diego region. The Center currently has over 100 staff working at 7 facilities across San Diego County.
PositionSummary
Under the supervision of the Senior Director of Programs, this role addresses the needs of transgender, nonbinary, agender, demi‑gender, gender‑fluid, two‑spirits, gender‑queer, intersex, and gender‑nonconforming individuals. The person will use peer‑to‑peer advocacy to assist client engagement with various organizations, navigating gaps and barriers faced by the gender‑expansive community. A deep lived‑experience navigating institutions, collaborative work with internal and external partners, and a client‑centered, resourceful approach are required.
ClientSupport
- Provide case management by creating individualized care plans to accomplish goals, remaining grounded with complex cases.
- Link clients to gender‑affirming and trans‑related care, treatment, and support.
- Navigate state and federally funded health insurance, educating clients about their rights and processes.
- Assist clients in filing grievances and appeals with insurance plans and managed care health agencies.
- Follow up on medical referrals and prior authorizations while maintaining HIPAA compliance.
- Help clients secure employment, financial security, and enrollment benefits, particularly those experiencing housing insecurity or displacement.
- Attend and facilitate support groups, including the Project TRANS Advisory Committee.
- Submit monthly program impact stories.
- Serve as an ambassador of Project TRANS at LGBTQ+ events and partners’ gatherings.
- Provide on‑site support to youth at both youth centers.
- Plan and execute Project TRANS events such as Transgender Day of Remembrance, Visibility, Trans & Enby Summit, Trans Pride Village, and gender‑affirming clothing drives.
- Maintain accurate documentation of clients’ goals and needs in internal data systems.
- Complete monthly narrative reports for the Center’s Quality Assurance team and external funders.
- Collaborate with the Project TRANS team to submit monthly program impact stories.
- Support administrative duties for Project TRANS, including calendar creation, inquiries response, and purchases for discussion groups.
- 1 year of experience providing case management services to transgender, nonbinary, and gender‑expansive communities.
- Proficiency with PC/Windows environments and Microsoft Office.
- Ability to manage multiple tasks with shifting priorities.
- Competence working with diverse populations.
- Competence using strength‑based, trauma‑informed, and motivational interviewing techniques.
- Fluency in English.
- Bachelor’s degree in a social service‑related field.
- 2 + years of experience providing case management services to transgender, nonbinary, and gender‑expansive communities.
- 1 + year of experience with clients living with mental illness and/or substance use disorders.
- Knowledge of current issues related to the LGBTQIA+ community.
- Fluency in Spanish.
- On‑site, in the office and with program participants in the field.
- Hillcrest Neighborhood, City of San Diego.
- Pay range: $23.00 – $25.00 per hour, commensurate with experience;
