Position: Social Services Manager (San Diego, CA - Onsite)

Position Summary

Reporting to the Director of Health and Social Services, the Social Services Manager will be directly responsible for the implementation, supervision, and coordination of activities for the Family Resource Coordination Team. In addition, with the collaboration of the Social Services Supervisors, the Social Services Manager will oversee the Patient Navigation Team and Social Services Team to ensure that department activities are carried out as planned within the organizational and data collection plan.

Functions of the Job
  • Directly and/or indirectly oversee the work and performance of the Patient Navigation, Family Resource Coordinators and Social Work, including being involved in hiring, promoting, disciplinary actions, employee evaluations, and any other human resources activities as required.
  • Develops and updates Patient Navigation, Family Resource Coordinators, and Social Work departmental policies and procedures, objectives, quality assurance program, safety, HIPPA regulations/standards in collaboration with the Director of Health and Social Services, as required.
  • Directly and/or indirectly oversee Patient Navigation, Family Resource Coordinators, and Social Work to ensure that department activities are carried out as planned within the organizational and data collection plan.
  • Consult with and advise Health and Social Services Department staff members as to the relationship of social, emotional, and cultural factors to health and medical care, and as to the availability of social services in the community.
  • Oversee performance of Social Services Department staff members to ensure patients receive quality and timely social services.
  • Ensure that program objectives, metrics, and goals are accurately tracked and met.
  • Demonstrated ability to establish and execute defined departmental goals and objectives to ensure compliance‑measured performance, and SYHealth strategic objectives.
  • Provide coaching, mentoring and leadership to motivate and empower staff.
  • Assist with quality control procedures to ensure operational processes are executed accurately.
  • Maintains a working knowledge of facility service areas including transportation, community characteristics, and geography.
  • Maintains a networking liaison with other organizations to address a broad range of social service needs for SYHealth patients. May be requested to represent the agency in contact with human service, health care, and community organization groups and individuals.
  • Attend meetings, teleconferences, and trainings; or ensure a knowledgeable proxy attends in the place of the program lead to ensure the transfer the information.

Job Requirements Education Required

Master's Degree in Social Work or related field.

Certifications/Licenses Required

Valid California driver’s license. Reliable transportation must be properly insured to meet Center’s guidelines.

Experience and Required Skills

Minimum two years in a supervisory/managerial position related to Social Services. Knowledge of community resources and their use to meet the needs of families. Project management skills. Ability to establish and maintain good interpersonal relationships. Work duties could include travel in San Diego county.

Verbal and Written Skills Required

Excellent oral and written communication skills. Bilingual preferred.

Equipment Used

Office equipment, including but not limited to, computers, phones, copy machines, fax machines.

Physical Requirements
