Shift differential for PM ($1.25/hr.), NOC ($1.50/hr.), and Weekend ($3.00/hr.)

Who you are:

* Current licensure by the California Board of Registered Nursing

* Current CPR certification

* Two years in long-term care with prior charge nurse experience preferred

* Experience with wound care and admissions strongly preferred

* Knowledge of CMS regulations and long-term care standards

* Strong assessment, organizational, and communications skills

* Ability to work independently and as part of an interdisciplinary team

Job Summary:

Coordinates and completes clinical admissions for new residents in a timely and accurate manner.

Essential

Job Duties:

* Perform comprehensive nursing assessments upon admission

* Review hospital records, discharge summaries, medication lists, and physician orders

* Initiate and document care plans based on resident needs

* Communicate effectively with physicians, interdisciplinary team members, residents, and families

* Ensure all admission documentation meets regulatory and facility standards

* Perform wound assessments, treatments, and dressing changes per physician orders

* Ensure compliance with infection control, safety, and quality assurance standards

* Maintain accurate and timely documentation in the electronic medical record (EMR)

* Participate in care plan meetings and interdisciplinary rounds as required

* Assist with audits, surveys, and regulatory compliance activities

* Support quality improvement initiatives related to admissions and wound care
