This is a full-time, on-site role for a Hospice Nurse based in San Diego, CA. The Hospice Nurse will provide compassionate care and support to patients and families, which includes pain and symptom management, administering medications, and coordinating holistic care plans. The role involves close collaboration with the interdisciplinary care team, ongoing patient assessments, educating families and caregivers, and maintaining accurate medical records.
The position requires adherence to hospice policies and ensuring compliance with regulatory standards.
- Proven nursing skills, including pain management, symptom control, and medication administration
- Excellent patient care coordination, planning, and critical thinking abilities
- Strong communication, empathy, and interpersonal skills to support patients, families, and team members
- Knowledge of hospice regulations, policies, and best practices
- Valid Registered Nurse (RN) license in California
- Certification in Hospice and Palliative Care (CHPN) is a plus
- Experience in hospice, palliative care, or home health settings is preferred
- Availability to work full-time on-site in San Diego, CA
- Proficiency in medical record documentation and familiarity with electronic health record systems
