• Provide direct care and emotional support to patients, their families, and caregivers.
• Apply critical thinking to assess patient needs and deliver appropriate care.
• Communicate effectively with patients and their loved ones.
• Collaborate with the interdisciplinary team to ensure high‑quality outcomes.

• Active California LVN license.
• Minimum one year of professional nursing experience (home health preferred).
• Current Basic Life Support (BLS) certification.
• Valid California driver’s license, auto insurance, and reliable transportation.
