As an On Call Licensed Vocational Nurse, you will provide direct patient care under the supervision of a Registered Nurse. You’ll follow individualized care plans, administer treatments, and collaborate with the interdisciplinary team to ensure comfort and dignity for every patient.
Key Responsibilities- Implement nursing care based on comprehensive assessments and care plans
- Accurately document patient services to reflect the plan of care
- Educate patients and families on diagnoses and self‑care plans
- Coordinate hospice services with the interdisciplinary team
- Use equipment and supplies effectively and efficiently
- Provide pain and symptom management, evaluating patient response
- Participate in quality improvement and professional development activities
- Valid California LVN license
- Minimum 1 year of hospice experience
- Experience with Home Care Home Base (HCHB) preferred
- Current CPR certification
- Valid driver’s license and insured, reliable vehicle
