Pay Range: $42.00 – $45.00 per hour

Schedule:

Monday - Thursday, 12:00 am - 8:00 am

About the Role

As an On Call Licensed Vocational Nurse, you will provide direct patient care under the supervision of a Registered Nurse. You’ll follow individualized care plans, administer treatments, and collaborate with the interdisciplinary team to ensure comfort and dignity for every patient.

Key Responsibilities
  • Implement nursing care based on comprehensive assessments and care plans
  • Accurately document patient services to reflect the plan of care
  • Educate patients and families on diagnoses and self‑care plans
  • Coordinate hospice services with the interdisciplinary team
  • Use equipment and supplies effectively and efficiently
  • Provide pain and symptom management, evaluating patient response
  • Participate in quality improvement and professional development activities
Qualifications
  • Valid California LVN license
  • Minimum 1 year of hospice experience
  • Experience with Home Care Home Base (HCHB) preferred
  • Current CPR certification
  • Valid driver’s license and insured, reliable vehicle
