Licensed Vocational Nurse (LVN) - Home Health Assignment Duration

This is a 13-week contract position.

Monday to Friday, with weekend rotation and days off during the week to prevent overtime. Shifts are 8 hours per day.

We are seeking a dedicated Licensed Vocational Nurse (LVN) for a home health position in San Marcos, CA. The ideal candidate will have recent home health experience and be familiar with Home Care Home Base (HCHB) systems. The position includes routine visits and requires the successful nurse to accumulate 32 points per week.

• Conduct routine home health visits to provide patient care.
• Maintain accurate and timely documentation of patient visits and care provided.
• Coordinate with healthcare teams to ensure comprehensive patient care.
• May be required to assist in other territories within the facility's service area, approximately a 60-mile radius.

• Valid LVN license in the state of California.
• Recent home health experience is required.
• Experience with Home Care Home Base is preferred; candidates without HCHB experience may be considered at a reduced rate for initial training.
