Hospice Clinical Manager; RN

Pay range (depending on experience): $130,000-$150,000

Schedule/Shift
:
Mon‑Fri 8am‑5pm

• Receives case referrals and reviews patient information to determine hospice/home care needs. Assigns appropriate hospice personnel as needed and conferences with the medical director regarding eligibility.
• Reviews and evaluates each case through home visits, conferences, record reviews, and clinician services; discusses impressions, guides clinicians, and promotes high‑quality home care delivery.
• Reviews patient medical diagnosis, prognosis, medications, procedures, and clinical course.
• Assists clinicians in establishing immediate and long‑term therapeutic goals and in developing a plan of care.
• Attends case conference meetings with hospice personnel to facilitate coordination of care and interdisciplinary discussion.

• Registered nurse with current state licensure.
• Previous experience in a hospice/home care setting and at least two years of management or supervisory experience.
• Excellent observation, nursing judgment, and communication skills; understanding of hospice philosophy and end‑of‑life care issues.
• Knowledge of pain and symptom management principles.
