Job Specific Details and Tasks

Work Schedule:

Monday - Friday 6:00am-9:00pm with one-hour lunch; occasional Saturday for Health & Wellness events.

  • Coordinate patient care in collaboration with a wide array of healthcare professionals; facilitate optimal outcomes in relation to clinical care, quality and cost effectiveness.
  • Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.
  • Perform physical exam and health histories.
  • Provide health promotions, counseling, and education.
  • Administer medications, wound care, and other personalized interventions.
  • Direct and supervise care provided by other healthcare professionals.
  • Accountable for making patient care assignments based on scope of practice and skill level of assigned personnel.
  • Recognize adverse signs and symptoms and quickly react in emergency situations.
  • Communicate and collaborate with a diverse group of people to inform the healthcare team of plans, actions, and to deliver patient/family education.
  • Make referral appointments and arrange specialty care as appropriate.
  • Maintain a level of productivity comparable with that of other individuals performing similar services.
  • Participate in peer review and performance improvement activities.
  • Practice aseptic techniques and comply with infection control guidelines.
  • Apply safety procedures and function with awareness of them.
  • Perform efficiently in emergency patient situations following established protocols, remaining calm and documenting events.
  • Apply an awareness of legal issues in all aspects of patient care and endeavor to manage situations with reduced risk.
  • Participate in the implementation of the Family Advocacy Program.
  • Exercise awareness and sensitivity to patient/significant others' rights as identified within the MTF.
  • Maintain an awareness of responsibility and accountability for own professional practice.
  • Engage in continuing education to meet professional growth.
  • Actively participate in the command’s Performance Improvement Program and review patient care delivery.
  • Provide in-service training to clinic/ward/unit staff members.
  • Attend and participate in various meetings as directed.
  • Perform timely, accurate, and concise documentation of patient care.
  • Operate and manipulate automated systems such as ADS, MHS Genesis and maintain DOD email account; respond to emails and voicemails.
  • Maintain documentation of all treatment provided in accordance with clinic directives.
  • Provide services within the scope of competencies and basic contract duties.
  • Manage care for patients of the Health and Wellness Department within the Health and Wellness and Disease Prevention domain, promoting effective education, self-management support, and timely health care delivery.
  • Collect, analyze, interpret, and evaluate data to assess trends and target the population.
  • Assess education needs of patients and families and provide clear instructions to promote self-management.
  • Provide highly coordinated nursing care interventions using established VA/DoD Clinical Practice Guidelines.
  • Participate as part of a multidisciplinary team: assess health promotion risk; analyze health data; develop content for educational programs.
  • Coordinate and deliver reports to Health and Wellness teams; consult and collaborate with existing departments and community programs.
  • Provide primary, secondary, and tertiary levels of intervention to promote effective education and timely health care delivery.
  • Educate patients on disease processes, impact of disease on overall health and wellness, and apply strategies such as motivational interviewing.
  • Work with all clinical teams as a resource on patient education and disease prevention.
  • Plan visits, summarize post-visit reviews, and provide follow-up contact to ensure compliance with recommendations.
  • Assist patients in setting goals, progressing, and identifying barriers when treatment goals are not met.
  • Conduct formal/informal group and individual educational and counseling interventions, including timely telephone follow-up consultations.
  • Evaluate achievement of learning objectives and provide appropriate follow-up.
  • Provide health promotion and disease prevention counseling for patients, family members and groups.
