As a Home Health RN Case Manager, you will play a vital role in delivering high‑quality, patient‑centered care in the comfort of the patient’s home. This role is ideal for a nurse who values autonomy, strong clinical judgment, teamwork, and making a meaningful difference in the lives of patients and families.

• Provide skilled nursing care to patients in the home setting
• Complete patient assessments, care planning, and ongoing case management
• Coordinate care with physicians, therapists, caregivers, and the interdisciplinary team
• Educate patients and families on disease management, medications, safety, and treatment plans
• Monitor patient progress and adjust care plans as needed
• Ensure timely and accurate documentation in accordance with Medicare, state, and agency guidelines
• Support quality outcomes, patient satisfaction, and continuity of care

• Home Health experience preferred

• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational skills
• Ability to work independently while staying connected to a supportive team
• Reliable transportation, valid driver’s license, and auto insurance
• Commitment to compassionate, patient‑first care

• Home Health experience preferred
