Com For Care  Home Health Care (North San Diego) is looking for a compassionate and skilled LVN to join our dedicated home health team in San Diego, CA. This is a rewarding opportunity to make a real difference in patients' lives by delivering personalized, one-on-one care right in the comfort of their homes. • Provide skilled nursing care to home health patients in accordance with physician-approved care plans
• Perform wound care, medication administration, injections, and other clinical procedures
• Monitor and document patient health status, progress, and changes in condition
• Educate patients and family members on disease management, medications, and home care techniques
• Coordinate with RNs, physicians, and interdisciplinary care teams to ensure continuity of care
• Maintain accurate and timely clinical documentation in compliance with state and federal regulations
• Conduct home visits across the North San Diego service area on a scheduled basis

• Valid California LVN license in good standing
• Current CPR/BLS certification
• Prior home health, skilled nursing, or clinical experience preferred
• Strong knowledge of wound care, medication management, and patient assessment
• Excellent communication, documentation, and organizational skills
• Reliable transportation, valid driver's license, and auto insurance
• Compassionate, self-motivated, and able to work independently in a field-based setting
