Home Health Nurse RN or LVN

Join Sevita as an LPN / LVN and provide compassionate, person-centered nursing care in home and community-based settings. In this Healthcare & Medical Services role, you'll deliver direct patient care, administer medications, monitor health status, and document in EHR systems while collaborating with an interdisciplinary team. Responsibilities

• Provide direct nursing care to individuals in home and community-based settings
• Administer medications and treatments per provider orders and company protocols
• Monitor, document, and report changes in patient status using EHR systems
• Support and implement individualized care plans developed by the clinical team
• Educate individuals and families on health conditions, medications, and self-care
• Collaborate with RNs, physicians, therapists, and support staff to coordinate care
• Assist with ADLs and promote safety, dignity, and independence for each person served
• Respond to emergencies and follow appropriate clinical and safety procedures
• Maintain accurate, timely clinical documentation and required reports
• Participate in ongoing training, quality improvement, and compliance activities
