Bilingual Complex Care RN | Telehealth & Care Coordination

Alignment Health seeks a Complex Care RN in San Diego, CA to coordinate care for high-risk Medicare Advantage members in a virtual care setting.

You will build trusted relationships with members and caregivers, perform medication reconciliations, and lead transitions of care with a multidisciplinary team.

Require ADN, CA RN license, BLS, and bilingual Spanish/English. This role offers growth within a Care Anywhere model and impactful, patient-centered care.
