Reports to: Program Managers of Safe-Sleeping and Director of Client Services
FLSA: Full-time (40 hours/week), Non-exempt (hourly) plus benefits
Schedule:
- Sunday - Thursday, 6:30 am - 3:00 pm (Friday - Saturday off)
- Sunday - Thursday, 9:00 am - 5:00 pm (Friday – Saturday off)
Salary: $27.12/hour

Under the direction of the Program Managers of Safe-Sleeping and Director of Client Services, the Case Manager plays a pivotal role in our mission by assisting homeless individuals in finding and securing stable housing. This position requires a compassionate, resourceful, and highly organized individual who can connect clients with available housing options, navigate complex housing systems, and provide ongoing support throughout the housing placement process.
The Case Manager will work closely with clients to identify their unique needs and barriers, develop personalized housing plans, and advocate on their behalf to access appropriate services and resources.
- Conduct comprehensive assessments of homeless individuals to determine their housing needs, preferences, and eligibility for housing programs
- Identify and access available housing resources, including shelters, transitional housing, and permanent housing options, and assist clients in finding suitable and affordable housing.
- Guide clients through the application and placement process for housing programs, ensuring all necessary documentation is submitted accurately and in a timely manner.
- Develop and implement individualized housing plans for clients, addressing barriers such as income, mental health, substance use, and legal issues.
- Advocate on behalf of clients to access supportive services, benefits, and community resources necessary for housing stability.
- Provide clients with information and education about their housing rights, responsibilities, and budgeting skills to promote long-term housing stability.
- Prepare and submit regular reports on client progress, program outcomes, and housing trends to support program evaluation and funding requirements.
- Collaborate with other service providers, community agencies, and landlords to expand housing options and support services for homeless individuals.
- Maintain accurate and confidential client records, progress notes, and documentation of housing placements and outcomes.
- Perform other tasks as assigned by department
- Adhere to all local, state, and federal guidelines and laws. (i.e., HIPAA, OSHA, FLSA)
- Answer all incoming calls from clients and various providers in polite and professional manner
- Respond to all email requests within 72 hours

Required Qualifications , Skills, and Abilities- Bachelor's degree in social work, psychology, human services, or a related field preferred
- Ability to work holidays and varied hours if necessary
- Previous experience working with homeless populations or in housing advocacy required
- Knowledge of local housing resources, housing laws, and social services.
- Strong communication, interpersonal, and problem-solving skills.
- Ability to work independently and as part of a team.
- Compassionate and nonjudgmental approach to working with homeless individuals and families.
