Job SummaryUnder general supervision, the Patient Engagement Specialist I recruits patients into the program’s system of care and supports the planning, development, implementation, and evaluation of patient engagement services in accordance with contractual and departmental requirements.
Responsibilities- Develop, identify, and provide referrals both within and outside of FHCSD and effectively monitor and track such referrals.
- Effectively communicate with and recruit new patients into the clinic’s or program’s system of care, engaging with community members and patients from diverse social, cultural, and economic backgrounds.
- Leverage community resources to raise awareness of FHCSD, establish and maintain good working relationships with community partners, and connect new and existing patients with FHCSD services.
- Act as an effective, productive, and collegial team member.
- Participate in and support the planning, development, execution, and evaluation of patient engagement plans.
- Investigate leads for patient recruitment and engagement opportunities.
- Perform other duties as assigned.
- Produce accurate and timely written work such as plans, flyers, and reports.
- Use computer resources and the required applications to meet organizational demands.
- At least 1 year of experience in sales, patient engagement, community education, community health or outreach programs.
- Associate’s Degree with a focus on psychology, sociology, social work, communication, public health or a related field is strongly preferred; relevant work experience may substitute for the degree.
- High School diploma or GED equivalent (or equivalent combination of education and experience).
- Valid California driver’s license, proof of minimum insurance coverage, and acceptable driving record.
- Willingness to travel between sites and other locations as an essential function of the job.
Skills and Abilities
- Maintain good interpersonal interactions with clients and co‑workers in a multidisciplinary community health team serving a diverse, multicultural population.
- Multi‑task, adapt to changing priorities and ensure accuracy in a fast‑paced, high‑workload environment.
- Basic computer literacy and ability to input information into a database and use Microsoft Office Suite.
- Fluency in both English and Spanish (read, speak, and write).
- Effective communication skills, including reflecting, active listening, and paraphrasing.
- Intermediate ability to investigate leads for patient recruitment and engagement opportunities.
- Intermediate written and verbal communication skills.
- Knowledge of community resources, health and social service systems in San Diego County, and ability to establish working relationships with community partners.
- Knowledge of current science and practices related to the health topic for the assigned program.

Salary range: $25.00 – $30.93 per hour
