The San Diego LGBTQ Community Center is seeking a Case Manager (HOY) to provide case management and community services to HOY participants under the Youth Housing Services Manager. You will support program operations, referrals, and outcomes, with a focus on LGBTQ youth and related equity goals.
You will conduct intakes, assess needs, coordinate services, and document progress in HMIS, helping youth access mental health, educational, and career resources.
