Singles Team, Transitional Bridge Shelter

At Father Joe's Villages, shelter is more than providing a safe place to sleep. It is an opportunity to help individuals build stability, reconnect with resources, and take meaningful steps toward permanent housing.

The Transitional Bridge Shelter provides short-term shelter and supportive services for individuals experiencing homelessness. Through housing-focused case management, resource navigation, and coordinated support, we help guests overcome barriers and move toward long-term housing stability and self-sufficiency.

Our Case Managers play an important role in that journey. Every day, they build trusting relationships, help clients navigate complex systems, connect individuals to critical resources, and support progress toward housing and personal goals.

What Success Looks Like

This is a direct-service role that combines case management, housing-focused planning, resource coordination, advocacy, and crisis support. Successful Case Managers are compassionate, organized, and comfortable working with individuals experiencing homelessness, behavioral health challenges, substance use disorders, financial hardship, and other barriers to stability.

Success in this role means building strong relationships with clients, helping individuals access services and resources, maintaining accurate documentation, and supporting measurable progress toward housing and self-sufficiency goals. You'll help clients overcome barriers, improve stability, and move closer to permanent housing and long-term success.

The Role
  • Conduct intake interviews and comprehensive assessments to identify client strengths, needs, goals, and service priorities.
  • Develop, implement, monitor, and update individualized service plans focused on housing stability, income growth, long-term self-sufficiency, and changing client needs.
  • Maintain an assigned caseload and provide ongoing case management support to help clients achieve established goals.
  • Connect clients to housing resources, public benefits, healthcare services, behavioral health treatment, employment programs, substance use treatment, and community-based support.
  • Assist clients with applications for Social Security benefits, disability programs, public assistance, and other income support.
  • Provide advocacy, problem-solving support, crisis intervention, and service coordination to address immediate and ongoing client needs.
  • Participate in multidisciplinary meetings and collaborate with internal teams and community partners to support coordinated service delivery.
  • Complete timely and accurate case notes, assessments, referrals, and service documentation in HMIS and other required systems.
  • Complete required trainings and policy acknowledgments and perform other duties as assigned.
  • Bachelor's degree in Social Work , Psychology, Sociology, Human Development, or a related field; or an equivalent combination of education and experience.
  • Minimum one (1) year of experience working with underserved, vulnerable, or at-risk populations in a social services, housing, behavioral health, or related setting.
  • Experience working in homeless services, shelters, housing programs, outreach, or human services environments preferred.
  • Valid California Driver's License required.
  • Knowledge of trauma-informed care, motivational interviewing, strengths
    -based engagement, and person-centered service approaches.
  • Understanding of homelessness, housing resources, public benefits, crisis intervention practices, and community-based services.
  • Ability to assess client needs and coordinate services across multiple systems.
  • Strong relationship-building skills and the ability toestablishtrust while maintaining professional boundaries.
  • Ability to remain calm, professional, and effective in crisis situations and emotionally challenging environments.
  • Strong organizational, documentation, and follow-through skills.
  • Ability to manage multiple clients, competing priorities, and deadlines effectively.
  • Experience using Microsoft Office applications and electronic case management systems, including HMIS.
  • Ability tomaintainconfidentiality and handle sensitive information appropriately.
  • Commitment to serving individuals with compassion, respect, empathy, empowerment, and dignity.
Work Schedule

This is a full-time position that combines shelter-based case management, client engagement, resource coordination, housing-focused planning, and participation in multidisciplinary team activities. While the typical schedule is Monday through Friday, occasional schedule flexibility may be required to support client needs, program operations, and organizational priorities.
