San Diego Youth Services seeks a Case Manager in the Youth Homelessness Program to be based in San Diego. This is a full-time & non-exempt position reporting to the Program Manager.

We provide safe places to live and long-term solutions through shelter, foster homes, community centers and housing to more than 16,000 youth every year at 100+ community and school locations. CONTEXT OF THE POSITION:The Youth Homelessness Program provides specialized wrap-around assistance to youth at-risk of or experiencing homelessness. This position is responsible for providing comprehensive trauma-informed case management services to youth. This position will include but is not limited to providing case management, independent living skills development, housing support, safety planning, linkage to outside resources, and additional individualized services to Transition Aged Youth (TAY) 18 - 24 years old.
Provide supportive services focused on obtaining and stabilizing housing. Manage participant motivation, skill building and permanency; housing system navigation and removal of barriers.
- Contribute to the integrated trauma-informed approach for youth, adhering to and upholding the rigorous standards of San Diego Youth Services as mandated by its mission, purpose, guiding principles and standards of quality.
- Assist youth in moving in and out of the coordinated entry system including administering the TAY VI-SPDAT to participants, if needed.
- Provide advocacy, information, referral, case management, independent living skills development, mental health support, co-occurring services, crisis intervention, employment readiness, education services, financial planning, permanency planning and other trauma-informed direct services to current/former homeless youth.
- Establish rapport with transition-aged youth from diverse backgrounds and experiences. Link youth with resources that are appropriate to their individualized needs.
- Develop Vision Plans, provide coaching and resource navigation services in areas relating to the development of independent living skills, educational and vocational services, health and wellness, and crisis intervention.
- Monitor participant progress and maintain accurate records including data entry into approved databases and reports.
- Maintain confidentiality and privacy standards of service partner protected health information and other applicable information and material in accordance with agency and center policies, and applicable regulations and laws including HIPAA.
