Jewish Family Service of San Diego (JFS) is seeking a compassionate, mission-driven Care Manager to join the Supporting Our Survivors (SOS) San Diego team. This full-time Care Manager role supports Holocaust survivors through person-centered, trauma-informed care management, including client intake and assessments, care coordination, crisis intervention, resource navigation, service planning, and ongoing short- and long-term support. The position also requires accurate documentation and data entry in care management databases to support grant reporting, program outcomes, and quality services.
Using an outcome-oriented, person-centered, trauma-informed approach, the Care Manager meets with clients in their homes and communities to assess needs, connect them with local resources, coordinate appropriate services, and provide ongoing support that helps each client attain and maintain the highest possible level of independence, stability, and self-sufficiency.
The SOS team provides culturally and linguistically appropriate support through integrated services for vulnerable aging Holocaust survivors, including health and mental health navigation, direct financial assistance, food access, transportation coordination, home care support, and connection to community-based senior services.
In this nonprofit human services role, you will use professional social work, care management, and resource coordination skills to build trust with clients, help ensure benefits and supports are in place, and make a meaningful difference for older adults across San Diego. The Care Manager plays an integral role on the SOS team by supporting program participants, documenting services and outcomes, and helping share impact with grantors and agency leadership.
This position reports directly to the SOS Program Manager and may also work with the Director of Holocaust Survivor Programs.

- Holocaust Survivor Assistance Program, Kavod SHEF (Survivors of the Holocaust Emergency Fund) and community supported holiday deliveries. - Assess each client utilizing social work skills and complete tracking tools in Database
- Determine, best fit for client within program eligibility requirements.
- Use assessment tools and obtain back-up documents per Grant requirements.
- Claims Conference appropriate, obtain all information and releases needed.
- If homecare is needed through grants, complete the necessary forms. Set up and monitor homecare after obtaining approval from program manager.
- If financial assistance is needed through grants, discuss with Program Manager to assure that the survivor is in the lowest income bracket or has an unmet need.
- Document and monitor all submissions for financial assistance.
- Attend quarterly Advisory Committee meetings and share limited details as requested.
- Provide on-going Care Management including crisis intervention, advocacy and communication with family and home care vendors.
- Attend program related meetings including agency’s case consultation meetings, grant progress meetings, and staff meetings.
- Plan and coordinate social events with the Program Manager within budget and grant guidelines.
- Obtain notes of thanks/appreciation from participants and write up short paragraph on their background and the assistance JFS has provided for sharing with funders.
- Enter all information in databases and other tracking sheets as needed.
- Participate in community Holocaust events.
- Complete requests for Kavod SHEF and other grants (such as Blue Card applications)
- Work on additional assignments, as appropriate.
