BASIC FUNCTION:

The School-To-Career Case Manager participates in the preparation, administration, and control of federal program projects and activities; create, manage, and maintain detailed participant files and program reports according to federal procedures and regulations using the Customer Information Services and Reporting System (CISRS).

Overview of the Role:
The School-to-Career Case Manager will work with students ages 14–22 who have disabilities. This is a 10 month, 8 hour position that requires flexibility and the ability to travel throughout the San Diego Unified School District (SDUSD). The main check-in location is on Polk Avenue, but the role includes regular travel to multiple school sites across the district. The Case Manager will primarily support programs: TPP (Transition Partnership Program), Work Ability I, and/or Post Secondary Placements.

This role involves extensive report writing, case management, and procedural oversight.
