About the Role:We are looking to recruit enthusiastic and hardworking, post qualifying social worker within our Transition Team in the Wakefield district.
You will work closely with our key partners in the children's teams and the CTLD health team, to carry out person led assessments with children and adults from the age of 16 years who are transitioning from children's services into adult social care.
The team consists of 5.5 social workers. As a social worker on the team, you will be passionate about making a difference, and ready to embrace change. Working Monday to Friday 8:30am - 5pm and to 4.30pm on a Friday.
