The Street Health Outreach Supervisor leads a team of outreach professionals who meet people experiencing homelessness where they are – on the streets, in encampments, at shelters, and throughout the community. This is a hands-on leadership role that supports staff through challenging situations, changing conditions, and safety concerns while reinforcing compassionate, effective outreach practices.

Working closely with healthcare, behavioral health, housing, and community-based partners, the Supervisor ensures that outreach services are coordinated, responsive, and focused on meaningful outcomes for the people served.

The Role Guide staff through complex client situations, barriers to engagement, and changing field conditions.

Coordinate daily outreach activities and help ensure appropriate staffing, coverage, and follow-through.

Reinforce trauma-informed, person-centered, and culturally responsive approaches to outreach.

Monitor program activities, services outcomes, and other measures of effectiveness.

Develop and maintain collaborative relationships with healthcare, behavioral health, housing, and community-based service providers.

Represent Street Health outreach services in community meetings and cross-agency collaborations.

What You Bring Valid California driver’s license required.

CPR Certification or ability to obtain CPR Certification Bachelor’s degree in Social Work, Public Health, Human Services, Healthcare Administration, or a related field. An equivalent combination of education and experience may be considered.
5+ years of experience providing outreach, case management, care coordination, community-based services, or related services to individuals experiencing homelessness or other vulnerable populations.
2+ years of supervisory experience, including responsibility for staff coaching, training, performance management, or team coordination.

Experience working in community-based settings with vulnerable populations.

Experience collaborating with healthcare, behavioral health, housing, and community-based service providers.

Compensation and Benefits Pay for this position is based on experience, qualifications, and internal equity. The anticipated starting pay range is $60,902/yr - $76,128/yr.
