Home Health Physical Therapist (PT) – San Francisco, CA

North Med Health is partnering with a well-established home health organization in the San Francisco Bay Area to hire a Home Health Physical Therapist (PT).

This role offers one-on-one patient care, a manageable territory, and flexibility within a supportive team structure.

This position is ideal for clinicians who enjoy autonomy, meaningful patient relationships, and the ability to deliver high-quality care in a home-based setting without excessive driving or unrealistic productivity expectations.

POSITION HIGHLIGHTS

• Setting: Home Health
• Schedule: Full-Time, Monday–Friday
• Territory: Small, well-defined coverage area
• Patient Population: Adult and geriatric patients
• Compensation: Highly competitive base salary ($130-190k) with benefits
• One-on-one patient care
• Minimal drive time within a defined territory
• Supportive clinical leadership and care coordination
• Long-term stability within an established organization

KEY RESPONSIBILITIES

• Provide in-home physical therapy evaluations and treatment
• Develop and implement individualized plans of care
• Educate patients and caregivers on mobility, safety, and recovery
• Coordinate care with interdisciplinary team members
• Maintain accurate and timely documentation
• Ensure compliance with home health standards and regulations

QUALIFICATIONS

• Licensed Physical Therapist (PT) or license-eligible in California
• DPT or Master’s Degree in Physical Therapy
• Home health experience preferred; training available
• Strong time-management and communication skills
• Comfortable working independently in the field
