Primary
Work Location:
MULTISITE:
Multisite staff work across several program locations throughout Alameda County, providing care and support wherever it is most needed. These sites offer a range of services, including post‑hospitalization medically supported shelter, medical respite care, skilled nursing, and caregiving support.
Job Summary:
Cardea Health is a nonprofit healthcare organization dedicated to providing compassionate clinical services to vulnerable populations. We currently have an opening for a Physical Therapist. Be an integral part of a team focused on the rehabilitation and recovery of patients. This may include providing care to individuals in unstable and/or non‑traditional housing environments, such as shelters or unsheltered settings. The Physical Therapist will be responsible for both the evaluation and treatment of their patient caseload.
Job Responsibilities & QualificationsResponsibilities:
- Perform initial and ongoing clinical assessments to identify patient needs, determine level of functioning
- Provide therapeutic treatments and evaluation of equipment needs as applicable
- Evaluate outcomes of care with appropriate discharge planning
- Apply concepts of infection control and standard precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease
- Accept clinical assignments that are consistent with education and competence to care for patients
- Complete appropriate documentation in a timely manner
- Meet mandatory continuing education requirements of the agency/licensing board and participate in in‑service programs
- Use effective interpersonal relations and communication skills to advise and consult with the family and other agency personnel as appropriate
- Promote company philosophy and administrative policies
- Demonstrate commitment, professional growth, and competency
- Other duties as assigned
Minimum Required Skills &
Qualifications:
- Education:
Bachelor of Science in Physical Therapy (or Doctor of Physical Therapy) from a CAPTE‑accredited program - Licensure:
Current California State License as a Physical Therapist and current California Driver’s License - Skills &
Certifications:- Therapy skills in alignment with generally accepted standards of physical therapy practice
- Strong interpersonal and communication skills
- Proof of current CPR certification
- Knowledge of durable medical equipment
Preferred Skills &
Qualifications:
- 1+ years of Physical therapy experience (preferred)
Physical Requirements:
- Occasional sitting or stationary work
- Frequent standing, walking, kneeling, stooping, and ascending stairs
- Occasional lifting and carrying up to 20 lbs
- Frequent pushing and pulling
- Frequent reaching high, low, and level
Work Environment:
- In‑person onsite at a supportive housing facility. Constant work around and/or with people OR Interaction with patients, clients, and external customers
