Key Responsibilities
* ** Individualized Care:
** Evaluate residents and design evidence-based treatment plans tailored to short-term rehab and long-term care goals.
* ** Clinical Excellence:
** Deliver high-quality therapy sessions that focus on restoring mobility, strength, and independence.
* ** Collaborative Planning:
** Work alongside the interdisciplinary team to enhance resident quality of life and participate in care conferences.
* ** Mentorship:
** Assist in the supervision of PTAs and provide education to nursing staff regarding safe mobility and restorative care.
* ** Accurate Documentation:
** Maintain thorough evaluations and daily notes in accordance with California state standards.

Qualifications
* Current, valid
** Physical Therapist (PT) License
** in the state of California.
