Care Manager

At the Institute on Aging (IOA), we are on the forefront of revolutionary healthcare models, reshaping the way people age in place. We are seeking a mission-driven Care Manager to provide comprehensive, community-based case management to older adults and adults with disabilities so they can remain safely at home with dignity.

• Conduct comprehensive and ongoing reassessments (psychosocial, physical, mental health, environmental, spiritual).
• Develop and implement client-centered Community Living Plans aligned with program guidelines.
• Conduct home visits, hospital and SNF visits, and escort clients to medical or other appointments as clinically indicated.
• Identify, arrange, and monitor community services, with strong knowledge of Medicare, Medi-Cal, and entitlement programs.
• Provide crisis intervention, advocacy, therapeutic support, and problem-solving.
• Meet with clients at least monthly (more often as indicated); continuously update care plans.
• Document all case management activity within 24–48 hours and maintain accurate, compliant charting.
• Monitor service quality and evaluate Purchase of Service needs.
• Maintain strong communication with physicians, community providers, caregivers, and interdisciplinary teams.
• Assess clients’ ability to remain safely at home and coordinate placement when necessary.
• Participate in CQI initiatives, research/data collection, supervision, and team meetings.
• Uphold NASW ethical and legal standards and comply with all IOA regulatory requirements.
• Engage in continuing education related to case management, gerontology, and community systems.

• MSW or other relevant Master’s degree (MPH, MFT with specialized experience in psychology, counseling, geriatrics),
• OR
• BA/BS in Social Work or related field plus two (2) years of relevant social work experience demonstrating autonomous biopsychosocial assessment and care planning.

• Minimum one (1) year working with disabled and/or older adults.
• Strong understanding of medical and psychosocial needs of functionally impaired adults.
• Experience with mental/behavioral health and substance use disorders highly desired.
• Demonstrated case management experience within community healthcare systems.
• Exceptional communication skills with clients, caregivers, and interdisciplinary teams.
• Strong organizational skills, problem-solving ability, and computer literacy.
