The Mission of the San Francisco Department of Public Health (SFDPH) is to protect and promote the health of all San Franciscans. SFDPH strives to achieve its mission through the work of two main Divisions - the San Francisco Health Network and Population Health. The San Francisco Health Network is the City’s only complete system of care and has locations throughout the City, including Zuckerberg San Francisco General Hospital and Trauma Center, Laguna Honda Hospital and Rehabilitation Center, and over 15 primary care health centers.
Under general direction, the Clinical Pharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. The essential functions include:
The essential duties of this position include but are not limited to:
- Interviews and examines patients for drug evaluation; checks on drug adherence and side-effects; discusses symptoms and effects of prescribed drugs with patients and maintains a drug history on patients interviewed.
- Consults with physicians on drug usage and the drug regime of specific patients particularly hypertensive, diabetic, and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients.
- Conducts both formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence‑based medication therapy and toxicity, etc.
- Participates in the development and revision of medication use policies and guidelines.
- Compounds drugs, prepares, and dispenses medications, checks pharmacy stock, and keeps records.
The 2454 Clinical Pharmacist also performs other related duties as assigned.
Nature of positions: Positions are located throughout the Department of Public Health, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), Community Primary Care and Mental Health Clinics, as well as San Francisco County Jails. Some positions may require employees to work directly with individuals within locked jails and psychiatric facilities, the homeless population and/or individuals who have or are carriers of infectious/communicable diseases.

Minimum Qualifications
License: Requires possession of a valid Registered Pharmacist license issued by the California State Board of Pharmacy. (Please attach a copy of license);
- A. Completion of six (6) months of a pharmacy residency program;
OR - B. Completion of six (6) months of a post‑graduate pharmacy fellowship program;
OR - C. Two (2) years of professional level experience as a Clinical Pharmacist (equivalent to class 2454 Clinical Pharmacist within the City and County of San Francisco);
OR - D. Possession of a Board Certification in a pharmacy specialty area.
One year of full‑time employment is equivalent to 2,000 hours (2,000 hours of qualifying work experience is based on a 40‑hour work week).
