Job Summary

Under the Department of Pharmaceutical Services, the Ambulatory Clinic Pharmacist provides pharmaceutical care to patients at all ambulatory care pharmacy locations within the UCSF Health System, and may also be asked to participate in pharmaceutical care in other sectors of the Department as needed. The Pharmacist performs a wide range of functions to ensure that drugs prescribed and used by patients are safe and effective, including obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drugs of choice and therapeutic plans, ensuring access to medications, educating patients on proper use, and ensuring patients understand cautions, adverse reactions, and interactions related to their therapy.

The Pharmacist will be part of an interprofessional team consisting of physicians, advanced practice providers, nurses, and other clinic staff, and may also oversee pharmacy technicians to facilitate insurance authorizations, prepare and dispense prescription medications, and manage clinical activities and the Patient Management Program in collaboration with UCSF Specialty Pharmacy. This position requires flexibility to orient and work at all UCSF Health locations depending on operational needs, as applicable.

Department Description

The Department of Pharmaceutical Services at UCSF has long been nationally recognized as one of the founding fathers for clinical pharmacy services, which was established in 1965. Today, the department continues to lead the profession through the integration of advanced technology & robotics, comprehensive pharmaceutical care and clinical services. The Department of Pharmaceutical Services provides a full range of services for patients cared for by the Medical Center at UCSF and its providers and payors: drug procurement, storage, and security;

safe, accurate and timely preparation and distribution of drugs; clinical pharmacy services; development of drug use policies; drug information services; and participation in technology assessment and quality assurance programs. The Department is affiliated with the UCSF Department of Clinical Pharmacy, an academic department within the School of Pharmacy, responsible for teaching professional courses in the Pharm.

Required Qualifications
  • Doctor of Pharmacy degree (Pharm

    D) or equivalent degree as determined by the National Board of Pharmacy.
  • Must possess a current license in good standing to practice pharmacy in the State of California.
  • Current certification for Basic Life Support (BLS) from the American Heart Association.
Preferred Qualifications
  • Completion of an ASHP-accredited PGY1 Pharmacy Residency, or at least 3 years of equivalent experience in ambulatory care.
  • Completion of an ASHP-accredited PGY2 Ambulatory Pharmacy Residency in relevant area, or at least 3 years of equivalent experience in ambulatory care.
  • Advanced Board certification (e.g., BPS).
  • Experience with navigating billing options for pharmacist care.
  • Experience precepting and evaluating pharmacy learners (e.g., residents and students).
License/Certification
  • Must possess a current license in good standing to practice pharmacy in the State of California.
  • Current certification for Basic Life Support (BLS) from the American Heart Association.
