Job Summary
The Department of Pharmaceutical Services provides a full range of services for patients cared for by the Medical Center at UCSF and its providers and payors. These services include drug procurement, storage, and security; safe, accurate and timely preparation and distribution of drugs; clinical pharmacy services for patients; development of drug use policies within the Medical Center; drug information services for clinicians and patients;
and participation in technology assessment and quality assurance programs. The Department of Pharmaceutical Services is affiliated with the UCSF Department of Clinical Pharmacy, an academic department within the School of Pharmacy, which has primary responsibility for teaching professional courses in the Pharm.

Under the Department of Pharmaceutical Services, the Inpatient/Acute Care Services Per Diem Pharmacist provides pharmaceutical care to patients at all acute care pharmacy locations within the UCSF Health System.
Duties include:
obtaining medication histories, reviewing and assessing medication profiles, providing drug information to patients and health care providers, collaborating with clinicians on drug choice and therapeutic plans, ensuring access to medications, educating patients on proper medication use, and ensuring patients understand cautions, adverse reactions, and interactions related to their therapy. The pharmacist also engages in duties to maintain an efficient and secure pharmacy distribution system such as purchasing, storing, securing, evaluating dispensing quality, compounding, and furnishing medications and devices.
Supervised pharmacists may assist in overseeing pharmacy technicians, student interns, residents, and colleagues. Pharmacists have no formal supervisory responsibilities or overall management of organized pharmacy programs.
- Graduated from an accredited school of pharmacy and possess a Doctor of Pharmacy degree (Pharm
D) or equivalent. - For graduates in a non‑US jurisdiction, pharmacists must have earned a Foreign Pharmacy Graduate Examination Committee (FPGEC) Certification.
- Doctor of Pharmacy Degree (Pharm
D). - Completion of an ASHP‑accredited PGY1 Pharmacy Residency, or at least 3 years of inpatient/acute care services pharmacy experience.
- Hold an advanced practice board certification (e.g., Board of Pharmacy Specialty certificates, AAHIVP, etc.).
- Experience participating in pediatric and adult emergency situations.
Active license to practice Pharmacy in the State of California; must be in good standing with the California State Board of Pharmacy. Active American Heart Association CPR Certification.
