We are looking for a compassionate, organized, team‑oriented individual to fill the role of Part‑Time or Per‑Diem Hospice Social Worker for the city of San Francisco, the Peninsula, and all surrounding regions.
In this essential position you will work closely with the Director of Nursing, clinical staff, and front office team to ensure coordinated patient care and provide support to our exceptional team of healthcare professionals. Your contributions will help us maintain our reputation for excellence and enhance the patient experience.

Duties & Responsibilities- Evaluate the need for medical social casework services to patients in their place of residence.
- Communicate patient needs to the physician and hospices interdisciplinary team.
- Plan quality medical social work services.
- Implement social work services to patients and their families in their place of residence.
- Evaluate medical social work services plan on an ongoing basis.
- Maintain assessment and progress documentation accurately.
- Attend and present appropriate patient‑related information at Interdisciplinary Team Meetings.
- Participate in the quality improvement program.
- Master of Social Work degree from a school accredited by the Council on Social Work Education.
- One year of social work experience in a healthcare setting required.
- Clinical experience with geriatric or terminally ill populations desirable.
- Current CPR certification.
