We are currently looking for a compassionate, organized, and team-oriented Home Health Registered Nurse (RN) – Level 1 to serve patients throughout San Francisco. In this essential role, you will collaborate closely with our interdisciplinary team, including nurses, therapists, and administrative staff, to provide individualized care and ensure high-quality patient outcomes.

ResponsibilitiesProvide skilled nursing services within scope of practice, following individualized plans of care
Perform nursing assessments, deliver care, evaluate patient response, and report changes to the Physician, Case Manager, and/or Director of Nursing
Administer medications, provide wound care, patient education, and disease management as ordered
Maintain appropriate clinical records and submit documentation on company devices per organizational policy
Coverage Area: Fixed Territory. You will be assigned a specific, compact coverage area (e.g., San Francisco proper) to minimize drive time
Follow all company policies and procedures
Perform other duties as assigned
Requirements0–1 years of clinical experience;
Home Health experience preferred but not required
Ability to travel within San Francisco and surrounding areas
Caring, compassionate personality with strong communication skills
Schedule: Monday – Friday only. No weekend or evening requirements.
Pay: $135,000 – $141,000 per year DOE
