Job Title: Home Health Registered Nurse (RN) – Case Manager Location: San Francisco, CA Job Type: Full-Time

Health Link Home Health & Hospice is a trusted provider across the Bay Area for nearly 20 years. We are currently seeking a compassionate and organized Registered Nurse (RN) to join our San Francisco team.

• Rideshare Support (SF Only):
  We know parking in the city is a challenge. We provide rideshare credits (Uber/Lyft) to get you to and from patient visits so you don’t have to battle for parking.

  We use AI-assisted charting software (Kinnser) to drastically reduce your documentation time.

• Target Compensation: $135,000 – $156,000 per year
• Sign-On Bonus: $10,000

• Manage Your Caseload:
  Perform comprehensive nursing assessments (OASIS), including Start of Care, Recertifications, and Discharges.
• Skilled Care:
  Administer medications, provide wound care (including Wound Vacs), and manage IV therapies.
• Coordinate:
  Act as the primary point of contact for physicians and the interdisciplinary team (PT/OT/ST) to ensure the care plan is working.
• Educate:
  Empower patients and families to manage their conditions safely at home.
• Document:
  Maintain accurate, timely clinical notes on company-issued iPads.

• LICENSE:
  Current, active California RN License is REQUIRED to apply. Applications without a valid CA license number will not be reviewed.
• Experience:
  
  Minimum 1 year of professional nursing experience required. Home Health experience is preferred, but we are willing to train strong hospital/SNF nurses.
• Travel:
  Valid California Driver’s License and reliable transportation are mandatory.

  Strong clinical judgment, organization, and the ability to work independently.
