Job Summary
The Full-Time Traveling Wound Care Nurse provides skilled, evidence-based wound care services under the direction and supervision of a licensed Mid-Level Provider (NP/PA) and collaborating physician, in accordance with company policies and applicable state and federal regulations.
The primary objective of this role is to deliver safe, ethical wound care while maintaining strict adherence to the scope of practice. This position requires flexibility, travel readiness, strong communication, and collaborative clinical decision-making.
This is a salaried, full-time role that includes travel and coverage responsibilities as part of the compensation structure.

- Deliver ethical, safe, and evidence-based wound care
- Recognize scope limitations and elevate care appropriately
- Maintain collaborative communication with supervising licensed providers
- Understand that coordination with a licensed provider is mandatory, as clinical decisions impact the supervising provider’s license
- Practice strictly within the scope permitted by individual licensure (LVN, LPN, or RN)
- Travel to assigned patient locations and see patients at least once weekly, or as clinically indicated
- Maintain an average productivity target of approximately 20 patients per week
- Perform comprehensive wound assessments within the individual scope of practice
- Document wound characteristics, including size, depth, drainage, tissue type, and surrounding skin
- Perform non-sharp wound care treatments in accordance with established treatment plans
- Implement wound dressing changes per mid-level orders
- Monitor for signs of infection, ischemia, deterioration, or complications
- Escalate all clinical concerns to the supervising mid-level provider
- No sharp debridement procedures are permitted unless specifically authorized within the RN/LVN/LPN scope and company policy
- Order DME
- Prescribe antibiotics
- Initiate new wound orders without mid-level collaboration
- All escalations must be collaborative and clearly documented
- Coordinate care with the supervising NP/PA prior to Treatment plan changes, Escalations, Suspected infections, Worsening wounds
- Communicate effectively with Patients and caregivers, Home health agencies, Skilled nursing facilities, Primary care providers
- Serve as an extension of the supervising licensed provider while recognizing scope boundaries
- Understand that all clinical coordination protects both patient safety and the supervising provider’s license
- Complete all SOAP notes on the same day of service by 9:00 PM PST
- Maintain accurate, compliant, and detailed documentation
- Respond daily to Company email, Internal chat messages, Medical record requests
- Document mileage and visit activity per company policy
- Notify scheduling and leadership of any barriers to completing visits
This is a traveling clinical role within California.
- Must be willing and able to travel to any assigned coverage area based on operational needs
- Travel to additional regions as operationally required
- Provide coverage support when requested
- Mileage reimbursement, flights, hotels, and approved travel-related expenses will be reimbursed per company policy.
- Flexibility and availability are required as part of full-time salaried expectations.
- Maintain an active, unrestricted nursing license (LVN, LPN, or RN as applicable)
- Practice strictly within the scope of licensure
- No independent prescribing
- No independent DME ordering
- No sharp debridement unless specifically authorized under licensure and company policy
- Participate in required training and quality improvement initiatives
- Maintain professionalism in all patient and agency interactions
- Provide timely notification of planned or unplanned time off to ensure continuity of care
- Full-Time
- Monday–Friday
- Typical clinical hours: 8:00 AM – 5:00 PM
- Actual schedule may vary based on patient volume and coverage needs
