DescriptionSutter West Bay Medical Group (SWBMG) is seeking an experienced Cardiology Nurse Practitioner to support our non‑invasive cardiology program, focusing on high‑volume pre‑transplant cardiovascular evaluation and longitudinal care for our liver and renal transplant programs. This role is embedded within a highly collaborative, multidisciplinary environment alongside transplant hepatology, nephrology, cardiology subspecialists, anesthesia, and surgery.
Clinical Focus- Provide comprehensive cardiovascular assessment and optimization for liver and renal transplant candidates
- Lead and manage pre‑transplant cardiology clinics, including risk stratification and longitudinal follow‑up
- Collaborate closely with liver and renal transplant teams, advanced heart failure and transplant cardiology, advanced cardiovascular imaging, interventional cardiology, and electrophysiology
- Coordinate diagnostic testing, guideline‑directed medical therapy, and peri‑transplant cardiovascular planning
- Participate in multidisciplinary transplant selection conferences
- Support quality improvement, protocol development, and program growth within Advanced Organ Therapy
- Contribute to teaching, research, and system‑level innovation
- Graduate of an accredited Nurse Practitioner program
- Board certified as a Nurse Practitioner
- Prior experience in cardiology, advanced heart failure, or transplant medicine is a plus
- Comfort managing complex, high‑acuity patients with multisystem disease
- Strong clinical judgment, communication skills, and commitment to patient‑centered care
