Travel RN - Case Management​/Utilization Review - Case Management

American Traveler is seeking an experienced RN for a Care Coordinator position focused on cardiac and heart transplant patient management, requiring a CA RN license and ambulatory care experience.

Job Details

• Ambulatory care setting within a large academic medical center,

• Care Coordinator unit focused on heart transplant and heart failure patients,

• Patient population includes heart transplant recipients and patients with heart failure,

• Responsibilities include immunosuppression monitoring, prophylactic antibiotic management, and monitoring for organ rejection and infection,

• Day shift schedule: 4x10-hour shifts, 7:30 AM - 6:00 PM,

• Floating to other facilities within the same health system is required,

Job Requirements

• Active CA RN license required,

• Current AHA BLS certification required,

• Minimum 1 year of ambulatory care nursing experience required,

• Cardiac nursing experience with management of heart failure and/or heart transplant recipients required,

• Experience with Phoenix EMR preferred,

• Experience managing other organ transplant recipients preferred,

• Working knowledge of immunosuppressive therapies preferred,
