This position is 100% in-person onsite and primarily supports patients in San Francisco with occasional support in Oakland and satellite clinics. The Ambulatory Pediatric Heart Center Nurse Care Coordinator performs a variety of clinical, administrative, and coordination tasks to provide high‑quality, patient‑centered ambulatory care for pediatric cardiology patients.
Responsibilities- Coordinate case management for a panel of pediatric cardiology patients, including insurance authorizations, home health, DME, medication refills, and patient education.
- Maintain clinic flow: schedule patients, perform chart audits, comply with infection control and accreditation standards, and document assessments in APEX.
- Manage patient medication ordering and authorizations, ensure safe administration, and keep precise records in APEX.
- Coordinate lab requisitions, follow up on results, and communicate findings to providers and families.
- Answer patient inquiries via phone, email, and in‑basket, document conversations, and facilitate referrals to outside resources.
- Assess patient needs through observation and communication, identify red flags, and initiate appropriate actions.
- Plan and triage new referrals, manage provider schedules, and coordinate multidisciplinary care including CCS reporting.
- Implement individualized nursing care, perform routine diagnostic and therapeutic procedures, and manage medication administration.
- Facilitate continuity of care and communication among the healthcare team, patients, and families using APEX data.
- Participate in performance‑improvement activities, data collection, and cost‑effective practice recommendations.
- Engage in professional development, attend meetings, maintain licensure, and comply with time‑keeping policies.
- Perform additional patient‑care related duties as directed.
- Bachelor's Degree in Nursing (BSN)
- Five or more years of pediatric cardiac inpatient nursing experience
- Experience in an outpatient/ambulatory setting
- Knowledge of pediatric congenital heart disease
- Strong heart rhythm interpretation skills
- Strong central venous PIV insertion skills
- Experience performing bubble studies
- Flexibility to orient and work at all UCSF Medical Center locations
- Master’s Degree in Nursing (MSN)
- CCS paneled provider
- Experience with an electronic medical record system, EPIC/APeX strongly preferred
- Microsoft Outlook proficiency
- Current licensure as a Registered Nurse in California
- Current ACLS certification
- Current BLS certification
- Current PALS certification
