About the RoleWe are seeking a specialized Home Visit RN to join our concierge clinical team. The Home Visit RN is a field-based clinician who delivers in-person nursing care to our Concierge members in their homes when needed. Working in close collaboration with the provider, the Home Visit RN conducts clinical assessments, performs procedures, draws labs, administers treatments, and serves as the provider's eyes and ears when needed.
The visits may be independent or in conjunction with the provider virtually to do a more in depth assessment. Visits will occur within assigned coverage area.
This role is ideal for an experienced RN who is self-directed, clinically confident, and genuinely passionate about building therapeutic relationships with women across the full spectrum of health — from wellness optimization to complex chronic disease management to end-of-life support. We are looking to hire 4 RNs to cover the San Francisco Bay area:
One for San Francisco, San Jose, the Peninsula and the East Bay. You will cover all patients in your assigned region.
You will know your patients by name, by story, and by circumstance. You will not be rushed. Every visit is an opportunity to learn something that changes the care plan — and you will have a direct line to the provider to act on what you find.
Key Responsibilities Clinical Assessment & Monitoring- Conduct comprehensive and focused in-home nursing assessments, including vital signs, physical assessment, functional status, fall risk, and cognitive screening
- Perform phlebotomy and specimen collection for laboratory testing ordered by the physician
- Assess wound status, administer wound care, and document healing trajectory
- Monitor patients with chronic conditions — including hypertension, diabetes, thyroid disorders, and autoimmune conditions — and report findings to the physician in real time
- Identify early signs of clinical deterioration or new concerns and initiate timely communication with the physician
- Conduct post-hospital and post-ER home visits to assess recovery, reconcile medications, and support care transition
- Administer injections and infusions as ordered (e.g., B12, hormone injections, iron infusions, immunizations)
- Perform 12-lead EKG in the home setting
- Administer nebulizer treatments and other respiratory support as ordered
- Collect cultures, swabs, and other diagnostic specimens
- Perform point-of-care testing (urinalysis, glucose, INR) as ordered and within scope
- Provide individualized education on chronic disease management, nutrition, activity, and self-monitoring techniques
- Coach patients on symptom recognition and when to contact the care team
- Support patients in understanding their care plan, upcoming procedures, or specialist recommendations
- Reinforce the provider's clinical recommendations in a way that connects to the patient's daily life and goals
- Serve as a communication bridge between the patient and the provider— translating home observations into actionable clinical information
- Coordinate with the provider on follow-up visit scheduling, order placement, and care plan adjustments based on home findings
- Support advance care planning conversations and assist patients in documenting care preferences
- Document all home visit findings, interventions, and patient responses in the EHR within 4 hours of visit completion
