Pathways Home Health & Hospice is seeking a Registered Nurse (RN) Case Manager for Hospice care in San Francisco County. The ideal candidate will coordinate care for patients, conduct assessments, and develop individualized care plans. The position requires a current California RN license and a minimum of one year of nursing experience.
Full-time schedule with shifts from 8:30am to 5pm, including rotating weekends and holidays. A sign-on bonus of $9,000.00 is available, along with competitive hourly compensation ranging from $56.95 to $78.30.
