Under the supervision of the Administrative Nurse Manager, guided by the policies and procedures of the Ambulatory Care Center, and in collaboration with the physician and other clinical staff, the Clinical Nurse PD is responsible for providing safe, efficient nursing care to patients in an out‑patient, primary care practice. Triages patients and responds appropriately according to protocol and nursing assessment. The Clinical Nurse PD utilizes the nursing process (Assessment, Planning, Implementation and Evaluation) to provide safe and therapeutic clinical care.
The Clinical Nurse PD also provides clinical support to providers and participates as part of the health care team to assure coordination and continuity of patient care. The Clinical Nurse PD is responsible for triage on behalf of the practice both in person and through telephone and electronic communication. S/he participates in the development and implementation of educational programs and materials for patients and their family members.
S/he delivers direct patient care to a variety of patient populations. All components of nursing care will be individualized to meet the patient's physiological and psychosocial development. The Clinical Nurse PD will play a critical role in the patient experience including optimizing clinic flow, providing in and outreach to patients and working collaboratively with the team to create a patient centered environment.
S/he is responsible for daily clinical operations and maintaining a safe environment for patients and staff. The Clinical Nurse PD consistently projects a professional and courteous demeanor, helps establish and maintain the highest service standards for the practice and complies with the Medical Center House and Telephone Standards. This employee will utilize computer skills in Apex and other UCSF computerized programs.
This individual supports the standards of the policies and procedures of the practice and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice. Embraces and consistently role models the UCSF concepts of values: PRIDE (Professionalism, Respect, Integrity, Diversity and Excellence) and AIDET (Acknowledge, Introduce, Duration, Explanation, and Thanks) throughout all interactions with patients, family members, and other employees, both in person and on the phone.
The Clinical Nurse PD must have flexibility to cover multiple UCSF medical center primary care and multi‑specialty clinic sites.
- Identifies the needs of patients through observations of disease symptoms, emotional state, cultural and socioeconomic background and response to medical regimen and treatment.
- Collects and organizes data from the client, encouraging them to present their problems or symptoms through active listening and open‑ended questions.
- Assesses each client’s problem(s) based on auditory, verbal, and emotional cues, as well as information extracted from the medical record.
- Triages and responds or refers inquiries appropriately. Recognizes and reports specific physical and emotional conditions/needs/concerns of the patients to the appropriate members of the health care team. Alerts physician of unusual observations of patients to facilitate immediate attention, e.g. chest pain, shortness of breath. Demonstrates ability to provide appropriate care based on the needs of a specific individual, including that individual’s age.
