About the RESET Center, operated by Connections

Located in San Francisco’s South of Market neighborhood, the RESET Center provides an alternative destination for individuals who are found to be publicly intoxicated (due to alcohol and/or drugs) and would otherwise be transported to the emergency department or jail.

In collaboration and partnership with local law enforcement and public health agencies, the RESET Center aims to effectively divert intoxicated individuals away from the criminal justice and/or healthcare systems to improve outcomes, reduce systemic burden, and support connection to needed resources within the community.

What You’ll Do

The Registered Nurse (RN) provides nursing assessment, monitoring, and protocol-driven intervention for adults experiencing acute intoxication within a low-barrier sobering environment. The RN performs abbreviated nursing triage per standardized procedure, which assesses behavioral and physical health needs, and documents in accordance with harm-reduction and trauma-informed best practices. This role ensures safe oversight, supportive care, and protocol-guided referrals or transfers for individuals who may otherwise be transported to an emergency hospital or jail.

  • Perform abbreviated nursing assessments, including vitals, impairment/intoxication levels, and withdrawal risk screening.
  • Recognize signs of medical or psychiatric emergencies and initiate EMS or ED transfers when clinically indicated.
  • Provide supportive care, including hydration, nutrition/snack support, wound care, and medication administration per standing orders for minor injuries or symptoms.
  • Assess and monitor patients through the sobering process, noting changes in behavior, cognition, or level of consciousness.
  • Offer education, motivational engagement, and compassionate communication regarding substance use, health risks, and harm-reduction strategies.
  • Utilize trauma-informed and de-escalation approaches to respond to escalating behaviors.
  • Conduct incident debriefs and complete required clinical documentation following escalations or emergency outcomes.
  • Collaborate, as appropriate, with EMS, law enforcement, and emergency department teams as part of alternate destination protocols.
  • Support center-wide safety and risk mitigation practices across the multidisciplinary team.
  • Participate in required onboarding and ongoing clinical training; maintain competencies and attend required meetings.
  • Perform all other duties as assigned within the scope of practice.
What You’ll Bring
  • Graduation from an accredited nursing program.
  • Unrestricted, active license as a Registered Professional Nurse through the California State Board of Nursing.
  • Basic computer literacy, including the ability to document in an electronic health record (EHR).
  • Ability to obtain and accurately record vital signs.
  • Strong attention to detail and ability to complete documentation in a timely manner.
  • Ability to successfully complete Basic Life Support (BLS) certification during the first week of employment.
  • Must obtain Overdose Prevention and Narcan Administration training within 30 days of hire.
  • The company has a mandatory vaccination policy. All successful applicants must be fully vaccinated, including showing proper documentation, or otherwise be exempt pursuant to the company’s exemption process prior to their start date as a condition of employment.
