The Nurse Coordinator serves in a navigation role and directs care to the patients referred to the Pediatric Heart Center (PHC), primarily coordinating care within the Fetal Cardiovascular Program. This clinician is experienced and highly skilled with an in-depth knowledge and understanding of pregnancy complications and/or congenital anomalies, the major treatment modalities, symptom management, surgical care and patient education. Under general supervision of the Medical Directors of the Fetal Cardiovascular Program, the nurse works independently to coordinate the clinical care for women referred to the program and their fetuses/newborns.

The nurse will utilize a high level of clinical knowledge to screen patient referrals, triage telephone calls and emails from patients/families; review outside medical records, coordinate patient evaluations, perform duties consistent with follow up care for patients as requested by the PHC providers, ensure efficient patient care flow during clinics both at Mission Bay and at outreach sites, act as the liaison between the inpatient and outpatient nursing staff to ensure continuity of care for patients and their families, and assist in the coordination of planned deliveries of established patients and fetal surgical procedures.

They consistently project a professional and courteous demeanor and demonstrate an ability to participate as an integral part of the team to maintain the overall quality of care provided by the Pediatric Heart Center and the patients shared with the Fetal Treatment Center.

The nurse utilizes an interdisciplinary approach across the continuum of care with adherence to National Patient Safety Goals set forth by regulatory requirements. The nurse is responsible for bringing together the multidisciplinary efforts of the Surgeons, Radiologists, Perinatologists, Cardiologists, Advance Health Practitioners, Clinic Staff, Residents, Fellows, IP Nursing Staff, Genetics, Social Work, Pharmacy and all other ancillary and support departments to ensure a high quality, coordinated, and satisfying experience for the patient.

DUTIES

& ESSENTIAL JOB FUNCTIONS CLINICAL KNOWLEDGE/SKILLS MANAGEMENT
  • Assist in coordinating new patient referrals by conducting phone intake, triaging according to practice guidelines and reviewing PHC criteria to ensure patient is appropriate for the clinic.
  • Assesses, plans, implements, and evaluates appropriate interventions for patients referred to the PHC.
  • Integrates nursing theory, research, and knowledge of congenital anomalies.
  • Maintains knowledge base consistent with the advances in treatment related to fetal and pediatric diagnosis and therapy.
  • CLINICAL SKILL
  • Performs priority technical procedures required by the practice assignment and does so safely, legally, effectively, and efficiently.
  • Utilizes computer technology for, and maintains confidentiality of data collection, documentation, communication and research. Proficient in Apex.
  • Is competent, and participates, in research protocols as dictated by specific departments.
  • Assists in informed consent process, screening potential patients for clinical trials, assurance of adherence to protocol inclusion and exclusion criteria.
  • CLINICAL ORGANIZATION
  • Manages clinical situations with advanced skills, insuring the in corporations of patients/families' physical, psychosocial, and cultural needs.
  • Utilizes the nursing process in providing care for a group of patients in accordance with all Medical Center and Ambulatory Services Policies and Procedures.
  • Establishes and/or revise priorities for job/patient care activities based on acuity, visit volume, resource availability, patient/family preference, and scheduled procedures.
  • Seeks consultation, supervision, and/or assistance when unable to perform procedures effectively, safely, or independently.
  • PATIENT CARE MANAGEMENT
  • Functions as patient advocate and facilitates patient/family participation in decisions regarding their care. Is a resource to patients and families.
