Job Description

Under the direction of the Ward 86 Nurse Manager, the Registered Nurse (RN) will work in a primary care setting, delivering comprehensive nursing care within a multi‑disciplinary team structure. Responsibilities include, but are not limited to, providing direct patient care, administering medications, coordinating patient care, conducting assessments, and managing sexually transmitted diseases. The RN will also provide patient education and actively participate in quality improvement initiatives related to triage and other programs as required.

Attendance at relevant meetings will be expected as part of this role.

About Ward 86 Clinic

Ward 86, the HIV/AIDS Clinic at the Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG) campus has been a leader and innovator in HIV care since the beginning of the epidemic. Ward 86 is the largest clinic providing primary medical care for people living with HIV for the San Francisco Health Network (SFHN) and also provides preventive services to patients at‑risk for HIV infection, including pre‑exposure prophylaxis (PrEP).

The clinic serves approximately 2,500 patients living with HIV, with over 24,000 clinical encounters a year. Services are offered through a comprehensive, multi‑disciplinary approach addressing primary medical care, urgent care, mental health evaluation and treatment, substance use assessment, HIV prevention services, and specialty care needs.

Responsibilities
  • Organize and provide nursing care to a patient population living with, or at risk of contracting, HIV/AIDS in an outpatient primary care setting.
  • Assist patients with navigating healthcare systems, including coordinating care with specialty clinics and outside agencies (e.g., pharmacies, social services, community programs).
  • Serve as patient advocate to ensure comprehensive care is delivered, including facilitating problem resolution within an interdisciplinary setting.
  • Facilitate patient medication management, including administration and patient education around treatment adherence, and coordinate with pharmacies and insurance providers.
  • Assess patients, analyze findings, plan nursing care, implement interventions, and evaluate care and outcomes; includes screening for sexually transmitted infections, assessing risk factors, and implementing interventions as needed.
  • Act as a central member of the interdisciplinary team and actively participate in the management of patients with multiple comorbid conditions and those at high risk of adverse outcomes.
  • Collaborate closely with clinic staff as a vital member of the nursing team.
  • Organize and provide direct nursing care for patients seeking clinic care and through EPIC patient calls and advice requests.
  • Organize and oversee transfer of patients to higher levels of care if needed.
  • Provide direct patient care for urgent care and primary care patients, including peripheral IV insertions, phlebotomy, medication administration, and IV fluid administration.
  • Administer long‑acting injectable medications.
  • Support triage workflows for medical referrals related to patient care plans.
  • Participate in data collection, reporting, clinical audits, and programmatic evaluations related to quality improvement and clinic‑based projects.
  • Participate in clinic meetings, nursing and staff meetings, and represent the clinic on committees and work groups as assigned.

  • One year of clinical experience with essential knowledge and abilities for the role.
