Join our dedicated healthcare team in San Francisco, CA, a vibrant city known for its diverse culture and iconic landmarks. As a key member of our transplant department, you will coordinate all aspects of patient care, both inpatient and outpatient, ensuring seamless service delivery and exceptional patient outcomes.
- Coordinate all aspects of care, including record review, medication refills, and patient education.
- Make referrals for medically necessary services, equipment, and supplies.
- Act as a liaison between administrative, clinical staff, and physicians within the transplant department.
- Assist and support patient discharge planning.
- Provide patient education regarding care and medications.
- Offer assistance to patients and physicians in clinics and on hospital floors as needed.
- Carry out patient care orders as directed by physicians and in accordance with hospital policy.
- Assist with patient admissions and discharges.
- BLS Certification
- California RN License
- Bachelor's Degree in Nursing
- Proficiency with EPIC Phoenix (strict requirement)
- Outpatient experience in EPIC
Monday to Friday, 5x8-hour shifts, with no weekends or holidays.
