On Lok PACE is seeking a Home Care Registered Nurse to provide direct skilled nursing services to participants enrolled in our PACE program, under the supervision of the Home Care Manager. You will develop and adjust home care plans and supervise geriatric aides and LVNs to ensure high-quality care.
You will collaborate with an interdisciplinary team to coordinate medical care, transportation, and family education, while maintaining compliance with nursing standards and regulatory requirements.
