Position: Ambulatory Nurse - Pediatric Cardiology (BCH-Mission Bay)

Under general supervision, guided by the policies and procedures of the Medical Center and Ambulatory Services, and in collaboration with providers and administrative management, the Ambulatory Pediatric Heart Center Nurse works autonomously, utilizing the nursing process, to assess, plan, coordinate, implement and evaluate patient care within the ambulatory setting. S/he will utilize a high level of clinical knowledge to fulfill patient needs, ensure efficient patient care, and provide patient education when necessary.

The Nurse Care Coordinator will act as a patient advocate and as a liaison between insurance companies, inpatient/outpatient services, patients/families and all other necessary healthcare services to manage and facilitate efficient and timely coordination of care. In addition, the Clinical Nurse Care Coordinator will assist in the development of patient care standards and practices.

The Pediatric Heart Center Nurse Care Coordinator is responsible for coordinating outpatient services and needs for a panel of pediatric cardiology patients and providers in the ambulatory setting. His/her responsibilities include both in clinic procedures, managing the Outpatient Heart Center clinic flow as well as assisting with insurance authorization and prior medication authorizations, contacting home health companies, assuring DME and other home supplies are adequate, assisting with medication refill requests, patient teaching and assisting in the overall coordination of timely patient care.

This position is 100% in-person onsite. This position support patient care primarily in San Francisco and as needed in Oakland and satellite clinics.

  • Case Management

    Contacting insurance companies, home health agencies, DME companies, parents, pharmacies, CCS etc. to coordinate timely care. Returning calls to pharmacies and parents to facilitate prescription renewals. Facilitating communication between parents, providers, and other necessary support services. Faxing and submitting paperwork in a timely manner. Assisting in creation/reorganization of teaching materials for patients. Patient education.

  • Clinic Coordination and Support

    Use clinical judgment when determining appropriateness of testing. Performs regulatory chart audits in compliance with Ambulatory Nursing Administration guidelines. Performs clinical activities in an accurate and timely manner. Complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, administrative and clinical practices. Complies with activities mandated by TJC/Title 22. Completion of annual review and trainings. Document appropriate patient assessment and needs in APEX.

  • Patient Medication Management

    The nurse will work as part of a multidisciplinary team to obtain medication authorizations, coordinate medication ordering and assure safe medication administration. Assist with prescription refills and medication authorizations. S/he will document and maintain a record of prescription refills, medication authorizations and all patient interactions via APEX.

  • Coordination of Lab Requisition s and Reports

    To assist providers in ordering and obtaining lab/other results, the nurse will:
    Coordinate completed lab requisition slips, send them to the appropriate outside lab or patient and follow up to confirm completion. Maintain a record of outside lab requisitions. Ensure follow up to completion of studies. Obtain, review, distribute outside lab results to appropriate providers. Set aside results that are outside the normal range and bring those reports to the attention of the ordering provider.

    Review testing results with patients and families as directed by providers.

  • Patient Support

    The nurse will answer phone calls/emails/CRMS/Inbasket messages and assist in addressing all patient care needs. The nurse will work within his/her scope of practice when answering all medical, medication, and treatment questions and escalates to the provider as appropriate. The nurse will bring communications regarding the patients' medical care or treatment to the attention of the provider and enter a note in the patient's medical record (via APEX) reflecting the conversation with the parent/patient.

    The nurse will facilitate letters for school/employment/other requests. The nurse will act as a patient ambassador by contacting families to ensure that they do not have questions or concerns after changes in medication, diagnoses or post-hospitalization. Participate as a member of the healthcare team to coordinate outside resources for pediatric cardiology patients. Follow up with families by phone and email as needed.

  • Assessment

    Identifies the needs and responses of patients to medical regimen and treatment through observation of symptoms, emotional state, and cultural and socioeconomic background.
